Nosocomial hepatitis A outbreak among healthcare workers and patients in a community hospital during an ongoing statewide outbreak

Objective:
The Tennessee Department of Health (TDH) investigated a hepatitis A virus (HAV) outbreak to identify risk factors for infection and make prevention recommendations.
Design:Case series.
Setting:Community hospital.
Participants:Healthcare workers (HCWs) or patients with laboratory-confirmed acute HAV infection during October 1, 2018–January 10, 2019.
Methods:HCWs with suspected or confirmed hepatitis A infections were interviewed to assess their exposures and activities. Patient medical records and hospital administrative records were reviewed to identify common exposures. We conducted a site investigation to assess knowledge of infection control practices among HCWs. Serum specimens from ill persons were tested for HAV RNA by polymerase chain reaction (PCR) and genotyped.
Results:We identified 6 HCWs and 2 patients with laboratory-confirmed HAV infection. All cases likely resulted from exposure to a homeless patient with a history of recreational substance use and undiagnosed HAV infection. Breaches in hand hygiene and use of standard precautions were identified. HAV RNA was detected in 7 serum specimens and all belonged to an identical strain of HAV genotype 1b.
Conclusions:A hepatitis A outbreak among hospital patients and HCWs resulted from exposure to a single patient with undiagnosed HAV infection. Breakdowns in infection control practices contributed to the outbreak. The likelihood of nosocomial transmission can be reduced with proper hand hygiene, standard precautions, and routine disinfection. During community outbreaks, medical providers can better prevent ongoing transmission by including hepatitis A in the differential diagnosis among patients with a history of recreational substance use and homelessness.

Clomazone dissipation in soil was examined in field and laboratory experiments. Field studies suggested a potential for injury to rotational crops such as wheat. Field half-lives were 5 to 29 d (average of 9 field studies was 19 d) for the Etowah clay loam (fine-loamy, siliceous, thermic typic Paleudult) and Lily loam (fine-loamy, siliceous, thermic typic Hapludult) soils. Clomazone had an average half-life of 34 d under lab conditions, indicating slower dissipation in the lab than in the field. Clomazone was moderately adsorbed to soil as indicated by a Kd of about 1, and a Kom of 52 in the two soils.

A 7.6-m core recovered from Lough Inchiquin, western Ireland provides evidence for rapid and long-term climate change from the Late Glacial period to the Mid-Holocene. We determined percentage of carbonate, total organic matter, mineralogy, and δ18Ocalcite values to provide the first high-resolution record of climate variability for this period in Ireland. Following deglaciation, rapid climate amelioration preceded large increases in GISP2 δ18Oice values by ∼2300 yr. The Oldest Dryas (15,100 to 14,500 cal yr B.P.) Late Glacial event is documented in this record as a decrease in δ18Ocalcite values. Brief warming at ∼12,700 cal yr B.P. was followed by characteristic Younger Dryas cold and dry climate conditions. A rapid increase in δ18Ocalcite values at ∼10,500 cal yr B.P. marked the onset of Boreal warming in western Ireland. The 8200 cal yr B.P. event is represented by a brief cooling in our record. Prior to general warming, a larger and previously undescribed climate anomaly between 7300 and 6700 cal yr B.P. is characterized by low δ18Ocalcite values with high-frequency variability.

A series of editorials in this Journal have argued that psychiatry is in the midst of a crisis. The various solutions proposed would all involve a strengthening of psychiatry's identity as essentially ‘applied neuroscience’. Although not discounting the importance of the brain sciences and psychopharmacology, we argue that psychiatry needs to move beyond the dominance of the current, technological paradigm. This would be more in keeping with the evidence about how positive outcomes are achieved and could also serve to foster more meaningful collaboration with the growing service user movement.

A unique atomic force microscope-based local thermal-mechanical analysis (LTA) technique was used to study the influence of room temperature aging on viscoelastic properties of ethylene-methacrylic (E/MAA) acid ionomers. This approach permits easy access to structural relaxation effects on viscoelasticity at a short aging time, for instance, before the occurrence of secondary crystallization differential scanning calorimetry (DSC) melting peak. A Burger model along with finite element method yields quantitative analysis of viscoelastic properties versus the aging time. Creep curves were obtained with LTA after various times of aging at room temperature upon cooling from the melt. Measurements were carried out at both 30 and 70 °C. The results reveal the effects of structural relaxation upon aging in the ion-rich amorphous region, the influence of secondary crystallites on the viscoelastic properties, and shed light on the processes associated with aging in E/MAA ionomers.

After an initial attempt to close arterial ducts by the transcatheter insertion of an umbrella device had left significant residual patency, further attempts at closure were made in 14 patients by the insertion of an additional umbrella device. The ages of the patients ranged from four to 11 years. Overall, 17 attempts were made to insert additional devices at intervals of between six to 26 months after the insertion of the first umbrella. Successful insertion of a device was achieved in two patients only at a third attempt. One patient required the insertion of a third umbrella to achieve complete occlusion of the patent duct. Complete closure of the residual patency was achieved in 13 of the 14 patients. The remaining patient has been scheduled for insertion of a third umbrella.

Self-healing ability is a fascinating property of some materials. Structural materials with self-healing ability require a subtle combination of flow and stiffness characteristics. A vivid demonstration of self-healing is the automatic sealing of the bullet hole after bullet penetration of the target material. The ionomer Surlyn® satisfies the needs of both structural requirements and self-healing to a certain extent. The viscosity of Surlyn is very high at room temperature, a favorable property for structural applications but unfavorable for self-healing. However, when it was heated to 100°C and then cooled back down to room temperature, its viscosity remains temporarily low at room temperature for a few minutes, a favorable condition for healing. Here we report the measurement of such short-time relaxation effects on flow properties in Surlyn using an atomic force microscope (AFM)-based local thermal mechanical analyzer (LTA).

The following is an edited version of the Von Hippel Award address, given by recipient W.W. Mullins at the 1995 MRS Fall Meeting. Mullins received the Materials Research Society's highest honor “for pioneering and profound contributions to the understanding of grain boundary motion, morphological stability, the structure of surfaces and interfaces, and flow and diffusion as stochastic phenomena.” Mullins is University Professor Emeritus of Materials Science and Engineering at Carnegie-Mellon University.

The surface compositions of three In-Pb alloys (1.42a/o Pb, 3.07a/o Pb, and 5.86a/o Pb) have been obtained as a function of temperature in both solid and liquid states using a scanning Auger microprobe. All data were obtained at temperature, from equilibrated samples in a region of about fifty degrees on either side of the liquidus temperature. Lead was found to be the segregating component in all cases. From these measurements, the heat of segregation has been calculated for all alloys in both states. In each alloy the magnitude of the heat of segregation declines significantly on melting although the absolute surface compositions are found to be continuous across the two phase solid-liquid field. These results are interpreted in the light of current segregation theories. Particular attention is paid to the role of elastic effects on solute segregation to free surfaces.
